What Makes This Role Different

What Makes This Role Different

  • Monday through Friday, 8-hour days. No nights, no weekends, no call.
  • An average of 5 encounters per day. We’ve built the caseload around the reality of palliative work — family meetings that run long, symptom management that requires nuance, and the emotional bandwidth this specialty demands.
  • A true interdisciplinary team. You’ll work alongside palliative physicians, chaplaincy, social work, and nursing — not in parallel, but together.
  • Protected time for what matters — documentation, debriefing, and the conversations that don’t fit neatly into an encounter.

What You’ll Do

  • Complex symptom management
  • Goals-of-care conversations with patients and families
  • Code status discussions, advance care planning, and POLST/MOLST completion
  • Family meetings, including those involving complex dynamics or surrogate decision-making
  • Coordination of transitions to home hospice, inpatient hospice, or palliative-supported discharge
  • Prognostic support and education for primary teams
