To:   All Employees currently participating in the FSA (Flexible Spending / Dependent Care Account)
 
The end of our flexible spending account plan year is fast approaching (the current FSA plan year ends 2/28/2013).  If you participated in the FSA this year, you have until 5/29/2013 to submit your claim(s) for eligible expenses incurred during the plan year of 3/1/2012 – 2/28/2013.    This is a “use it” or “lose it” plan.  Amounts do not roll-over.
 
Completed forms, along with receipt copies, should be sent directly to Admin America, (fax number is located on the bottom of the form).  
Reimbursement checks are processed once a month – the last Monday of each month.

Claim forms and information regarding eligible expenses can be found on the ADP Portal by clicking on the Home Tab and then selecting Resources from the drop down.  The forms are located under the Benefits Section.  If you have any questions, please call Human Resources at 770-274-0475.
